ALTERNATIVE CERTIFICATION District Instructions

The employing school district must complete one of the following:

An Application for Provisional Certificate (Joint Application).

An Application for Temporary Certificate (Joint Application). (complete only pages 4 & 5)

Once completed, please send signed originals to our office.
